Quality Assurance
Each CDDO is charged with ensuring that services provided in the CDDO catchment area are provided as specified and in a manner that is responsive to the person-centered support plan, offers opportunities of choice to the person served, and ensures that all person's rights are observed and protected. The DSNWK CDDO believes that it is the responsibility of each community service provider to develop, implement, and demonstrate their own processes for quality assurance and enhancement, which are reviewed at least annually by the CDDO Quality Assurance/Quality Enhancement Team.
The CDDO QA/QE utilizes a monitoring system that measures the quality of services of each Community Service Provider throughout the CDDO area. The Performance Indicator System, compromised of 15 quality indicators, has been operational since September 1998. This system functions with two groups of persons involved in the review process.
The Council of Community Members is the acting Quality Assurance Committee and a QA/QE Team, operating under contract, conducts the review process and reports their findings to the Council of Community Members/Quality Assurance Committee.
A Mission Statement for the QA/QE Team is as follows:
"Through a reasonable and equitable approach, the QA/QE Team shall monitor and report on the quality of services throughout the CDDO region, verifying compliance with applicable standards through an outcome based, continuous improvement process. Communication through the review process shall occur which will affect the highest level of improvement while holding providers of service accountable to the established quality standards."
The Performance Indicator System
Accountability and Timeliness - The service provider is accountable for following through with commitments made when planning for each individual.
Diversity - To have a well-rounded life, family members and/or community members are involved in planning and enriching the individual's life.
Use of Natural Supports - Community based services require the use of community facilities and access to all the services available to others in the community.
Community Inclusion - Involvement in the community's business, spiritual, and social aspects, both private and public.
Respect/Rights - Provides protection for vulnerable population.
Rights Restriction Process - Freedom from restrictive procedures can be overruled only when necessary for protection from harm and with the knowledge and concurrence of others who know and care about the individual.
Desired Lifestyle - Each person has the right to make decisions about where to live, with whom, and what daily activities to do.
Health/Safety - When protection from harm conflict with the person's desired lifestyle, the degree of risk is considered and as much freedom as is reasonable is provided.
Safety at Home and in the Neighborhood - Adequate community service requires a feeling of safety.
Background Checks for Employees - No service provider would knowingly endanger those they serve by hiring a person known to have caused abuse, neglect, or exploitation (ANE).
Site Checks for Health/Safety - The service provider follows regulations and applicable licensing standards and maintain service settings which promote health and safety.
Service Provider Accountability - Attestation of compliance demonstrates that the service provider has provided all the services that were purchased.
Abuse/Neglect/Exploitation (ANE) - ANE is responded to by the service provider. An allegation is investigated promptly and corrective action taken as needed.
Fiscal Responsiveness - The service provider has the right to prompt and accurate payment for the services provided.
Fairness - Compensation for work done. Each person who works shall be paid the minimum wage or a commensurate wage under a current federal wage and hour certificate.
